Composition: Enoxaparin Sodium
Indication: Prophylaxis of deep vein thrombosis which may lead to pulmonary embolism, ischemic complications of unstable angina, and non-Q-wave MI when currently administered with aspirin; prevention of thrombus formation in extracorporeal circulation during hemodialysis.
Dosage: Abdominal surgery: Initial dose given 2 hrs pre-op; SC inj 40 mg once a day 7-10 days; Max duration: 12 days. Hip/knee replacement surgery: Initial dose to be given 12-24 hrs post-op; SC inj 30 mg every 12 hrs; Hip replacement surgery: 40 mg once a day SC, initially 12 (+/- 3) hrs pre-op; Initial phase of thromboprophylaxis in hip replacement surgery: 40 mg once a day SC inj for 3 wks, max duration up to 14 days. Thromboembolic complications due to severely restricted mobility during acute illness: SC inj 40 mg once a day, 6-11 days, max duration up to 14 days. Unstable angina / non-Q-wave MI: SC inj 1 mg/kg every 12 hrs in conjunction with oral aspirin therapy (100-325 mg once daily) for 2-8 days, max duration up to 12.5 days. Deep vein thrombosis with or without pulmonary embolism: SC inj 1 mg/kg every 12 hrs for 7 days, max duration up to 17 days.
Contra Indication: Patients with bleeding diathesis, uncontrolled arterial HTN, or a history of recent GI ulceration, diabetic retinopathy, and hemorrhage. Elderly and children.
Precaution: Preg Risk: B.
Drug interaction: Hematomas. Intrahepatic hemorrhage, lumbar plexopathy.
Preparation: Pre-filled syringe 40 mg/0.4 mL x 1’s. Pre-filled syringe 60 mg/0.6 mL x 1’s
