INDICATION: Treatment of hyperkalemia resulting from acute & chronic renal failure
DOSAGE: Adult 15 g PO tid-qid. In case of vomiting & in patient w/ upper GI problems including paralytic ileus 30 g by rectal route suspended in 100 mL water, 2% methylcellulose or 5% glucose soln & left in the intestinal tract for 30 min-1 hr. Children Acute hyperkalemia 1 g/kg daily in divided doses. Maintenance: 0.5 g/kg daily in divided doses. Neonate 0.5-1 g/kg by rectal route, diluted as in adults.
ADMINISTRATION: Should be taken on an empty stomach: Powd may be taken w/ a small amount of water (30-50 mL) or made into paste w/ sweetened liqd, jam or honey. Avoid fruit juices/carbonated drinks that contain high K.
CONTRAINDICATION: History of hypersensitivity to polystyrene sulfonate resins. Obstructive bowel disease. Patients w/ serum K <5 mmol/L; conditions associated w/ hypercalcemia (eg, hyperparathyroidism, multiple myeloma, sarcoidosis or metastatic carcinoma). Oral administration & reduced gut motility (post-op or drug-induced) in neonates.
SPECIAL PRECAUTION: Monitor serum K, Na, Ca & Mg levels regularly. Pregnancy & lactation.
ADVERSE REACTION: Hypokalemia, hypercalcemia; symptoms of intestinal intolerance eg, nausea, vomiting, gastric irritation, anorexia, constipation & diarrhea, fecal impaction; GI concretions.
DRUG INTERACTION: Digitalis, cation-donating agents, non-absorbable cation-donating antacids & laxatives eg, Mg or Al hydroxide, sorbitol.
